What: cargo ship bulk carrier, the Integrity
Where: fringes of Australia’s Great Barrier Reef
When: May 19, 2012
Why: A damaged freighter The Integrity which suffered an engine breakdown drifted over Shark Reef Saturday night and is continuing to drift west towards the Outer Reef. The tow ship the Pacific Responder and two tugs are en route but are not expected to reach the Coral Sea until Sunday. They are racing to reach the damaged ship in time to prevent damage to the Great Barrier Reef. With no cargo aboard, and no pollution from the affected engine, the ship is adrift on open sea and ship management predicts that “there should be no damage to the great barrier reef.”

What: SA Roadlink bus en route from Gariep to Bloemfontein
Where: N1
When: 4:45
Who: 28 passengers, 5 fatalities
Why: The driver lost control of the bus apparently while swerving to avoid an animal, and crashed on the N1. Five people died in the crash. A special investigation of the cause is underway. The safety record of the bus company is under scrutiny now. Paperwork concerning roadlink buses which had been discontinued, impounded or involved in crashes in the past five years has been requisitioned from provincial and metropolitan traffic police departments.

Transport Minister Sibusiso Ndebele has conveyed condolences to the families of those killed in the crash.

“We want to convey condolences to the families of those killed in the road crash in the Free State this morning (16 May), and wish the injured a speedy recovery.

The South African Police, in conjunction with the Road Traffic Management Corporation (RTMC) as well as an independent team of accident investigation and reconstruction specialists, have commenced investigations into this crash. We call upon the investigation team to ensure that those responsible for these unnecessary deaths face the full might of the law. Officials from the Road Accident Fund (RAF) are also providing assistance to the affected families.”

“Over the past four months (January to April 2012), more than 17 343 un-roadworthy vehicles, mostly buses and taxis, have been discontinued from use as part of the new National Rolling Enforcement Plan (NREP).

During the same period, 4 067 297 vehicles and drivers were checked, 1 457 516 fines issued and 24 703 drivers arrested, including 7 017 for drunk driving, 378 for reckless and/or negligent driving, 908 for excessive speed, 391 for overloading, 12 975 for outstanding warrants of arrest, 446 for not being in possession of valid public transport permits, 279 for false documentation, 208 for not being in possession of valid driving licences and 2 101 for various other offences.”

“Although a slight year-on-year reduction in road deaths has been recorded, a great deal of work must still be done in line with the United Nations (UN) Decade of Action for Road Safety 2011-2020. Enlist as a Friend of the Decade of Action. Together, we can put an end to deaths and injuries on our roads.”

Where: Mount Prospect
Why: One of three dozen victims of a May 13, 2011 train crash in Mount Prospect took Metra to Chicago court Tuesday. The driver of the dump truck, Kazimierz Karasek, that was struck was killed at the scene.

The negligence lawsuit concerns Metra’s speeding train that was running without lights, horns, too many cars and inadequate braking. Rafael Ortiz who brought the charges was a passenger on the train who suffered permanent and temporary injuries to his head, body, mind, limbs and nervous system.

Where: Frontage Road
When: May 14, 2012 7:20 a.m.
Who: 27 injured, 29 passengers
Why: In front of T.J. Maxx and Chili’s, a bus in East Haven \stopped on Frontage road to let a passenger put his bike on the rack, when the bus was struck from behind by a restaurant grease truck. The bicyclist was knocked under the bus, and 26 others inside were injured. The bus was knocked 100 feet.

The bus driver stayed till the last injured was taken care of, concerned about passengers who take the bus daily.

The most serious injuries— the bicyclist and the truck driver plus five in the bus—were taken by ambulance to Yale New Haven Hospital and the Hospital of Saint Raphael. A firetruck with paramedics responded, as well as ten ambulances. A dozen of the injured took a transit bus to the hospital. They were not around when workers were cleaning up, and a fire started under the grease truck. The fire was quickly extinguished.

The section of Frontage road where the accident occurred is known to be treacherous.

Maritime Search and Rescue Center of Fuijian Province choreographed the rescue ships.

A crew has remained aboard to help drain the ship of water, but it is considered to be in stable condition, and its fuel valves are shut off. Passengers were taken to Qingzhou.

What: Schoolbus
Where: Stavropol Krai
When: May 13, 2012 12:20
Who: 4 fatalities, 5 injuries
Why: A schoolbus en route to a musical festival passed a double decker passenger bus before the passenger bus rear-ended them in a construction area.

Eighteen children and four adults were in the schoolbus traveling to a musical event where the 9 to 17 year old children were dancers. A ten and a thirteen year old and a 56 year old teacher died on the scene. A 39 year old chaperone died at the hospital. The injured were taken to hospitals in Pyatigorsk. Three of the injured children were moved to Moscow on Monday.

All of the children were wearing their seatbelts, but organizers say the police failed to provide escorts.

The schoolbus driver hit the brakes, and the double-decker bus slammed into the back of the schoolbus. The schoolbus was knocked over the barrier, and rolled into a ditch.

The MAN driver is being cited for ‘insufficient following distance and speed’ because he did not allow for enough room to stop. The MAN bus was en route from Nalchik to Moscow, and carried 20 passengers, none of whom were hurt.

A criminal case has been opened as a “Violation of traffic rules and operation of vehicles, resulting in the death of two or more persons” which incurs a punishment of up to seven years prison sentence.

At the event, Children of the Caucasus the victims of the crash were remembered with a moment of silence, and the festival was cancelled.
